Experience Requirements

• Specialty: Radiology / Cardiology (Echo Tech)
• Required years of experience: 2 years (based on "2YR/1ST TIMERS" note)

Certifications Required

• ACLS (Advanced Cardiac Life Support) - AHA or Canadian Heart & Stroke Foundation certification
• BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
• PALS (Pediatric Advanced Life Support) - AHA or Canadian Heart & Stroke Foundation certification
• NRP (Neonatal Resuscitation Procedure) - Current certification by AAP, AHA, or Canadian Paediatric Society

• State License (professional license copy required)
• IV Certification (if applicable)
• Management of Assaultive Behavior (MAB) (if applicable)

Background and Compliance Requirements

• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start, updated monthly
• Education Verification required for all candidates
• State-specific searches (Oregon Abuse/Neglect, Washington WSP/WATCH) as applicable
• Drug testing: 10-panel drug screen within 90 days of start (no instant tests)
• References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
• Employment application covering at least last 5 years (7 years for APN/Subcontractors)
• Skills checklist updated annually (if applicable)

Health and Safety Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Fit mask test (N95 or approved equivalent) required for travel candidates, updated annually
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
  • Negative TB test or chest X-ray within 12 months, updated annually
  • Measles (Rubeola), Mumps, Rubella: 2 MMR vaccines or positive titer, boosters or declination accepted
  • Hepatitis B: Vaccination series or signed declination; titers accepted
  • Varicella: 2 vaccines or positive titer, boosters or declination accepted
  • Flu vaccine during flu season or declination accepted
  • COVID vaccine or declination required
• Respiratory Safety Clearance (RespSafety) required annually for travel candidates

• Float policy acknowledgment required (may be required to float to other Providence facilities nearby)
