Job Opportunity: Home Health Occupational Therapist (Travel/Contract)
Location: Portland, Oregon
Specialty: Therapy/Rehabilitation (Occupational Therapy)
Contract Length: 13 weeks
Job Details
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1; may visit up to 6 patients per day
- Productivity Standard: 30 productivity points per day
- Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Accepted
- Weekend Work: Not required
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals: Accepted within a 50-75 mile radius at the same rate
- Pending License: Accepted
- Transportation: Traveler must have their own transportation
- Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
- OASIS Experience: Required
Certifications Required
- Basic Life Support (BLS) certification required
- Other certifications may be required based on clinical track (e.g., ACLS, PALS, NRP) but BLS is mandatory for this role
Facility & Work Environment
- Type: Home Health
- Attire: Business casual; scrubs allowed but no specific color required; no jeans
- Charting: EPIC Remote Client (laptop) and Rover (phone app) with OASIS documentation
- Parking: Free
- Mileage Reimbursement: IRS standard rates
Compliance & Documentation Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- State-specific background checks and abuse/neglect searches as applicable
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness to work
- Immunizations and health screenings required:
- Tetanus/Diphtheria/Tdap (within 10 years or declination)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination during flu season
- COVID vaccine or declination
- Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
- Respirator fit testing required for travel candidates (updated annually)
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application covering at least last 5 years of employment
Training & Competency Requirements
- Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Core modules for Oregon Home Health caregivers
- Specialty-specific modules as applicable
- Annual regulatory requirements
- Competency exams and medication/pharmacology tests as applicable
- Epic training modules required and must be completed within a week of orientation
Additional Notes
- Traveler may be required to complete a float policy acknowledgement (though floating is not required for this position)
- Must comply with all state-specific licensing and background check requirements, including multi-state licensure attestations if applicable
- Facility may call off up to one shift per pay period (every two weeks)
- Prior travel experience is not required for this position

This is a travel/contract opportunity for an Occupational Therapist with at least 1 year of skilled home health experience.
