Travel RN - Care Management

Location: Naples, Florida
Employment Type: Travel

Job Details

Contract Length: 13 weeks
Shift: Days (5 shifts per week, 8-hour shifts from 8:00 AM to 4:30 PM, rotating weekends)
Specialty: Registered Nurse (RN) - Care Management
Patient Ratio: Approximately 1:25
Environment: Fast-paced

Requirements

Active Florida state or multistate RN license
BLS certification
Minimum of 2+ years hospital care management experience
Experience with Cerner EMR system
Ability to manage discharge planning for all levels of post-acute care
Must be able to handle a high patient ratio and fast-paced environment

Responsibilities

Collaborate with interdisciplinary healthcare team to meet resource management goals
Assist in reducing physician practice variations
Enhance patient outcomes with focus on decreasing length of stay
Monitor cost-effective patient care across the continuum
Determine medical necessity of admissions and continued stays
Identify inappropriate or untimely services
Assess discharge planning needs for patients of all ages
Coordinate delivery of services to meet discharge needs
Facilitate timely and appropriate hospital discharges
Identify opportunities for patient care improvement
Provide documentation as required by regulatory agencies
