Job Opportunity: Occupational Therapist (Home Health)

Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks


Job Summary

The Occupational Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education.


Key Responsibilities

  • Evaluate and establish plans of care to meet patient needs in the home environment.
  • Coordinate care with other disciplines and obtain orders for additional services as needed.
  • Provide direct patient care based on established care plans and physician orders.
  • Apply knowledge of occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations, re-evaluations (30-day functional assessments), and treatment techniques. Modify treatment as appropriate, including frequency and duration of care.
  • Master scoring of CMS-required OASIS and collaborate with Quality team for necessary adjustments.
  • Maintain understanding of insurance requirements in Home Health.
  • Participate in multidisciplinary conferences to establish appropriate patient care plans and communicate with physicians and team members.
  • Document all skilled services, adverse events, and relevant communications in the medical record.
  • Participate in Quality Assurance Performance Improvement initiatives.
  • Manage and meet productivity standards.
  • Maintain accurate time logs and mileage records; follow agency policies for commuting and logging.
  • Communicate daily with scheduler and optimize continuity of care and travel routes.
  • Oversee services provided by Occupational Therapy Assistant and supervise CNA when applicable.
  • Participate in care conferences and plan admissions, OASIS time points, and discharges.
  • Maintain occupational therapy licensure requirements, including continuing education.
  • Adhere to ethical standards as defined by the occupational therapy licensure board and agency policies.
  • Participate in agency-provided educational opportunities.

Minimum Qualifications

  • Bachelor’s degree or higher from an accredited Occupational Therapy program.
  • Current license to practice Occupational Therapy in the State of Montana.
  • Valid driver’s license and appropriate auto insurance.
  • Required Experience: 2 years of previous Occupational Therapy experience preferred (use 2 years as minimum requirement).
