Job Description

• Must possess a valid degree and New York State licensure.
• Conduct in-person interviews as part of the hiring process.
• Obtain and review individuals' health histories.
• Examine and evaluate individuals' physical and neurological conditions.
• Perform and interpret tests and measurements to assess movement-related disorders.
• Develop and implement individualized physical therapy treatment plans, including goals and anticipated outcomes.
• Select, administer, and monitor therapeutic treatment modalities such as heat, cold, light, air, water, sound, electricity, massage, mobilization, and therapeutic exercise (with or without assistive devices).
• Conduct and supervise individual and group physical therapy activities.
• Observe and document individuals' progress, responses, and behavior; modify treatment plans as needed.
• Collaborate with Physical Therapist Assistants.
• Attend bi-weekly Care Conferences.

Required Experience

Background & Compliance Requirements

• Criminal background checks including SS Trace, County searches, Nationwide, Sex Offender Registry.
• Fingerprinting required onsite on the first day of orientation.

• 5-year criminal background check within 6 months of start.
• State-specific searches including New York All Counties Search.
• Medication Test/Pharmacology Competency Exam (for RNs and LPNs).
• Competency Exam required for all candidates.
• New York Statewide Central Registry Form Search (tracking purposes).

Health & Immunization Requirements

• Physical exam within 1 year of start.
• Negative TB test or chest X-ray (Qgold, Tspot, or 2-step PPD) within 12 months prior to start.
• Annual TB Questionnaire.
• Vaccination documentation or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (vaccination, titers, or declination)
  • Varicella
• Drug testing: 9-panel within 1 year of start.
• Seasonal flu vaccination or declination (2025-26 season).
• COVID-19 vaccine booster preferred but not required.

• Two professional references required.
• Medicaid enrollment documentation may be required depending on facility policies.
