Job Requirements

Note: Open to first-time travelers only if they have strong outpatient OT and neuro experience. Candidates without outpatient and neuro experience should not be submitted.

Required Documentation & Compliance

• Identification: Government-issued photo ID required (no paper copies except temporary receipts)
• Background Checks:

  • Within 6 months of start date, covering all locations lived/worked in past 7 years
  • Includes SS Trace, Counties, Nationwide, Sex Offender, FACIS Level 3, Education Verification
  • International searches if applicable
• Education Verification: Third-party verification required
• Licensure Verification: Primary source verification required
• Certifications: Must be current and verified (BLS, PAMCA, ACLS, PALS, NRP, etc. as applicable)
• Competency Exams: Unit-specific competency exams and medication/pharmacology tests as applicable
• Physical Exam: Completed within 1 year of start, confirming ability to meet essential job functions
• Fit Mask Test: Respirator fit test completed onsite (not required for non-clinical)
• TB Testing: Negative TB test or chest X-ray within 1 year of start
• Immunizations:

  • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer required
  • Varicella - 2 vaccines or positive titer required
  • Hepatitis B - Titer, series, or signed declination required
  • Flu vaccination or approved medical/religious exemption required during flu season
• Drug Testing: Negative 9-panel drug screen within 30 days of start
• Additional Health Documents: TB Questionnaire (updated annually)

Facility-Specific Requirements

• Completion and signing of facility policies such as:

  • Consent to Release Information
  • Floating Policy for Contracted Staff
  • Time and Attendance Policy for Contracted Staff
• Stroke and Trauma CEUs may be required and must be completed/maintained as specified
• EKG/Dysrhythmia competency exams may be required depending on unit
• Pass/Fail testing acknowledgment for RNs & LPNs as applicable
