Job Description

• Provide Occupational Therapy services in a home health setting
• Mileage reimbursed at IRS rate
• Valid driver's license and clean driving record required
• Locals allowed at the same rate

Requirements

• Minimum 2 years of experience in Occupational Therapy
• Valid and current driver's license required for all Home Health positions
• Must have a clean driving record
• Must be at least 21 years old (25 if transporting patients)
• Background check: 7-year criminal history check within a year of start, updated annually
• DMV driving record required with at least 5 years of driving history
• Must have minimum 3 years driving experience regardless of age

Documentation & Compliance

• I9 verification required
• Government-issued photo ID required
• E-Verify required
• Background checks including fingerprinting (Virginia VSP National Criminal Background Check)
• Physical exam within last 12 months
• Negative TB test or chest X-ray within last 12 months (or clearance letter if positive)
• Drug screen within last 12 months
• Flu vaccination required during flu season (10/1-3/31) with strict deadlines for documentation or approved exemptions
• COVID vaccination series preferred
• Professional references: 2 required
• Employment application/resume required
• Skills checklist must be updated annually

Certifications & Competencies

• Current BLS (Basic Life Support) certification required
• Other certifications (ACLS, PALS, NRP, EFM) as applicable based on unit or specialty (check with program management)
• Medication/Pharmacology competency exam required as applicable
• Competency exams must be completed within a year of start date
• Acknowledgement of pass/fail testing required for all clinical positions

Facility Specific Policies

• Confidentiality agreement, dress code, scrub colors, safety brochure, drug-free policy, and hand washing hygiene policy must be reviewed and signed prior to start
• Photo for badge must meet specific requirements (recent, professional, clear, neutral background)
• Virginia State Police Check must be completed and cleared prior to start for Home Health positions
• TB questionnaire and clearance letters must be updated annually
• Stroke education CEUs required for RNs in acute care (not applicable here but noted for reference)
