Position Details

Vaccination Requirements

  • Must be fully vaccinated for COVID-19 (per DC mandate)
  • COVID booster preferred but not required
  • Medical and religious exemptions or declinations for COVID vaccine are NOT accepted

Compliance and Documentation Requirements

  • Proof of I9 or attestation
  • Government-issued photo ID (must be carried during hours of service)
  • E-Verify or attestation
  • Background check: 7-year criminal history, sex offender, FACIS Level 3, education verification, and international searches as applicable
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated for renewals
  • Education verification through primary source (transcripts/diplomas not accepted as sole proof)
  • State nursing license verification (Washington D.C. license only, must be current and valid)
  • NURSYS verification for all RNs
  • Drug screening: 9-panel drug screen within last 12 months (instant acceptable)
  • Physical exam: Initial and updated annually, must meet physical demands of position
  • Fit mask test within one year of start, updated annually
  • Immunizations and titers required:
    • Tetanus/Diphtheria/Tdap (vaccine or titer; declination accepted with form)
    • Negative TB test or chest X-ray within 1 year
    • Rubeola (Measles), Mumps, Rubella (titer or 2 vaccines; booster if low titer; declinations only for medical/religious reasons)
    • Hepatitis B (vaccine, titer, or declination form)
    • Varicella (titer or 2 vaccines; booster if low titer; declinations only for medical/religious reasons)
    • Flu vaccine required annually (religious and medical exemptions accepted with approval and forms)
  • Additional health documents:
    • Eye exam including visual acuity
    • Color blind exam (mandatory, online exam not accepted)
  • References: Proof of 2 clinical references or hospital evaluations
  • Employment application/resume including licensure, certifications, education, and employment history

  • Orientation Safety Manual Acknowledgement required
  • Candidate must complete facility-specific forms such as position description acknowledgement and various declination/exemption forms if applicable
