Certifications Required
- BLS (Basic Life Support): AHA or Canadian Heart & Stroke Foundation certification
- ACLS (Advanced Cardiac Life Support): AHA or Canadian Heart & Stroke Foundation certification
- PALS (Pediatric Advanced Life Support): AHA or Canadian Heart & Stroke Foundation certification
- NRP (Neonatal Resuscitation Procedure): Current certification by AAP, AHA, or Canadian Paediatric Society
- Other Certifications: May be required based on clinical track (e.g., Chemotherapy, EFM, IV Certification, Management of Assaultive Behavior, NIH Stroke Certification, CPI) - verify with clinical track
Licensure
- State License copy required (RTs in CA require CPDH certificate)
- NURSYS verification required for RNs
- Allied professionals must meet licensure requirements per Allied Licensure Report
Background and Compliance Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Various state-specific searches and verifications as applicable
- Education verification required for all candidates
- Drug screen: 10 panel within 90 days of start (no instant tests)
- References: Minimum 2 supervisor references covering 1 year of employment in specialty (6 months for CNAs)
- Employment application covering at least last 5 years (7 years for APN/Subcontractors)
- Skills checklist updated annually (if applicable)
Health Requirements
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit mask test (annual, travel only)
- Immunizations/vaccinations:
- Tetanus/Diphtheria/Tdap (within 10 years or declination)
- Negative TB test or chest X-ray (annual update required)
- MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer or declination
- Hepatitis B vaccination or signed declination
- Varicella - 2 vaccines or positive titer or declination
- Flu vaccine during flu season or declination
- COVID vaccine or declination
- Respiratory safety clearance (annual for travel)
Training and Competency
- Medication Test/Pharmacology Competency Exam as applicable
- Competency exams required by specialty
- Color Blind Testing if required by unit
- Specialty-specific competency exams (e.g., Medication Aide exam for LTC CNAs in Oregon Assisted Living)
Facility-Specific Requirements
- Float Policy Acknowledgement required (may be required to float to other Providence facilities within reasonable distance)
- Badge photo (passport style) required
- Completion of Providence WorkRamp modules prior to start, including:
- Core modules for profession and region (Oregon-specific modules required)
- Specialty-specific modules for ER, L&D, and leadership roles (not required for X-Ray Techs)
- Annual regulatory modules required for all caregivers
- Epic training modules required (completion and transcript submission prior to moving to Paid Day Work status)

- Prior travel experience is not explicitly required for this position.
