Job Description
- Patient Population:
- 85% Pediatric Outpatient
- 15% Adult Outpatient
- 1 day Adult Acute Care
- Shift: Tuesday to Saturday, 5x8 hour shifts (8:00 AM - 4:30 PM)
- Tuesday to Friday: Outpatient
- Saturday: Acute Care
- Daily Patient Load: 12-14 patients
- Guaranteed Hours: 40 hours/week
- Facility Type: Short Term Acute Care
- Trauma Level: Level IV Trauma
- Charting System: Cerner
- Dress Code: Business Casual
- Parking: Free
Requirements
Experience
- Years of Experience Required: Minimum 1 year in specialty
- First-time Travelers: Accepted
- Prior Travel Experience: Not required
Certifications & Licenses
- BLS (Basic Life Support): Required (AHA only, AHA-RQI Provider Cards accepted)
- CCC-SLP (Certificate of Clinical Competence in Speech-Language Pathology): Required
- Other Certifications:
- ACLS, PALS, NRP, and others may be relevant depending on unit but not explicitly required here
Background & Compliance
- 7-year background check within 30 days of start
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- Fingerprinting required (valid Arizona fingerprint card)
- Drug testing (10 panel) within 30 days of start and updated annually
- Physical exam within 1 year of start, confirming ability to perform essential job functions
- Fit mask test (N95) within 1 year of start and updated annually (PAPR not accepted)
- Immunizations and titers required or declinations accepted for:
- Tetanus/Diphtheria/Tdap
- Measles (Rubeola), Mumps, Rubella (MMR)
- Hepatitis B
- Varicella
- Influenza (2025-26 season) or signed declination form
- COVID vaccine series or declination form
Documentation
- Valid state license for Speech Language Pathologist
- Employment application covering last 3 years with documented gaps
- Two professional references (one from most recent employer within 1 year)
- Facility-specific job description form to be completed prior to start
- Security question form and travel information verification form to be signed prior to start
- Photo for badge required
Additional Notes
- Locals accepted at the same rate as travel
- Return to office (RTO) restrictions require Unit Manager approval
- Facility allows calling off 6 times during the 13-week assignment
- Candidates must comply with facility-specific policies on professional attire, confidentiality, and substance abuse screening
This position offers a structured outpatient and acute care schedule in a teaching hospital environment with a focus on pediatric and adult speech therapy services.
