Job Opportunity: Therapy/Rehabilitation - Physical Therapist (Home Health)
Location: Shelton, Connecticut
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 13 weeks)
Job Description
- Provide physical therapy services through home visits.
- Participate in periodic office meetings.
- Average patient load: 5-6 patients per day.
- Patient visits include Start of Care and Follow Ups.
- Must have reliable transportation and a current driver's license.
- Mileage reimbursed at IRS standard rate (patient home to patient home; commute to/from first and last patient not billable).
- Must be willing to travel up to 75 miles from home clinic.
- Schedule: Monday to Friday, 8 AM to 5 PM (standard 5x8 schedule).
- Weekend rotation: None.
- On-call: None.
- Electronic Medical Record (EMR) system used: HCHB.
- Territory includes multiple towns in Connecticut (Hamden, Orange, West Haven, New Haven, North Haven, Woodbridge, Bethany, Cheshire, Wallingford).
- Local candidates accepted at the same rate.
Requirements
- Specialty: Physical Therapy in Home Health.
- Minimum 1 full year of recent Adult Home Health experience required.
- First-time home health travelers not accepted; must have recent home health experience.
- Certifications required: CPR and BLS (Basic Life Support) current and valid.
- Must have license in hand prior to start (pending licenses accepted only if expected within a few days).
- Must be willing to comply with all required background checks, health screenings, and facility-specific documentation.
Productivity Expectations
- Weekly productivity: 30 points per week (points assigned based on visit type and activities such as documentation and travel).
- Patient load typically equates to 6-8 points per day with 5-10 patients assigned daily.
Compliance and Documentation Requirements
Candidates must complete and maintain the following prior to and during assignment:
- Valid Driver’s License/State ID.
- Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
- Fingerprinting as required by state regulations (CT requires fingerprints prior to start).
- Education verification for degree applicable to profession.
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel).
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
- Completion of facility-specific compliance training, confidentiality agreements, HIPAA and privacy policies, and other required policy acknowledgments.
- References: Two professional references required.
- Work history verification: At least two most recent employers or last 7 years if only one employer.
Additional Notes
- Must have agency-required liability insurance.
- No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.
- Weekend and on-call requirements vary by location but are not required for this assignment.
- Candidates must review and sign new supplier packets at the start of assignment and annually if working more than one consecutive year.
This position is ideal for experienced physical therapists with recent home health experience seeking a travel contract in Connecticut with a structured weekday schedule and a defined patient load.
