Job Opportunity: Physical Therapist (Outpatient/Inpatient Swing Bed)

Location: Bremen, Indiana

Position Details

• Specialty: Therapy/Rehabilitation (Physical Therapy)
• Shift: 8:00 AM – 4:30 PM
• Call Requirements: None
• Weekend Requirements: 1 Saturday per month
• Holiday Requirements: None
• Work Setting: Primarily outpatient with some inpatient treatments (swing bed). No outpatient neuro population.
• Guaranteed Hours: Yes

Experience & Certifications

• Required Years of Experience: 3 years in Physical Therapy
• First-time Travelers: Accepted
• Required Certifications:

  • Basic Life Support (BLS)
  • State Physical Therapy License
• Dress Code: Gray scrubs or business casual
• Charting Systems: Redoc (Outpatient), Cerner (Inpatient)

Background & Health Requirements

• Background check covering the last 7 years, including all states lived/worked in, required within 90 days of start.
• Education verification or copy of diploma/transcripts required.
• Physical exam required.
• Negative TB test or chest X-ray within 1 year of start (2-step PPD or Qgold accepted; T-Spot not accepted).
• TB questionnaire required prior to start and annually thereafter.
• Immunizations or titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, series, or declination)
  • Varicella
• Negative 12-panel drug screen within 30 days of start (includes amphetamines, cannabinoids, MDMA, methamphetamines, buprenorphine, oxycodone, fentanyl, cocaine, methadone, tramadol, benzodiazepines, morphine/opiate).
• Flu vaccine or approved medical/religious exemption required prior to start (flu season October–May).

Additional Requirements

• COVID-19 vaccine required unless a medical or religious exemption is approved.
• Facility-specific documents and agreements must be completed prior to start.
• Passport-style photo required for badge.
• Completion of facility onboarding modules (approx. 8-10 hours) prior to start; paid hours for module completion. No modules required if worked with facility within past year.

Prior Travel Experience

• No prior travel experience required for this position.

This role is ideal for a Physical Therapist with at least 3 years of experience looking for a travel contract in outpatient and inpatient rehab settings with a stable schedule and minimal weekend/holiday requirements.
