Registered Nurse – Cardiac Rehab (Phase II) | Outpatient
Location: Quincy, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Overview
Support a high-volume outpatient Phase II Cardiac Rehabilitation program serving patients with cardiac diagnoses. Focus on patient monitoring during supervised exercise therapy, cardiac education, care planning, and care coordination.
Schedule
- 4 × 10-hour shifts per week
- Hours: 6:30 AM – 5:00 PM
Primary Responsibilities
- Monitor EKG rhythms and vital signs during supervised exercise sessions
- Assist patients with exercise equipment and ensure safe participation
- Provide cardiac education and wellness coaching
- Develop and document individualized care plans
- Set and track patient wellness goals
- Collaborate with the medical director and interdisciplinary care team
- Complete charting and documentation outside of direct patient care times
- Schedule patient appointments and follow-up visits
- Communicate with ordering providers regarding patient progress and care coordination
Unit Environment
Busy outpatient cardiac rehab program requiring active monitoring of EKG rhythms, vital signs, and patient activity. Agency RNs support both direct patient care and essential non-direct-care tasks including documentation and care coordination.
Requirements
- Active RN license
- BLS required
- ACLS preferred (may be required by start)
- Cardiac, telemetry, or cardiac rehab experience preferred
- Strong EKG monitoring skills
- Excellent documentation and communication skills
- Ability to work independently in a fast-paced outpatient setting
Experience
- Specialty: Registered Nurse (Cardiac Rehab)
- Required Years of Experience: Not explicitly stated; cardiac, telemetry, or cardiac rehab experience preferred (assume minimum 1+ years)

Certifications & Competencies
- BLS (Basic Life Support) required
- ACLS (Advanced Cardiac Life Support) preferred/possibly required
- NIH Stroke Certification required and must be updated annually
- Medication Test/Pharmacology Competency Exam (80% passing score or meet/exceed Angoff score)
- Color Blind Testing required
- Unit Specific Competency Exam required
- Core Part I, II, III required and must be updated annually
- OSHA Mandatory training required annually
Health & Compliance Requirements
- Physical Exam within 30 days of start, stating clearance to work without restrictions
- N95 Fit Mask Test within 12 months of start
- Tetanus/Diphtheria/Tdap vaccination within 10 years or signed declination
- Negative TB test or Chest X-Ray within 1 year of start
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
- 10-panel Drug Screen within 30 days of start, repeated annually (no rapid results)
- Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)
- COVID-19 Vaccination proof required prior to start
Documentation & Verification
- Government issued photo ID (2 forms of ID required)
- Background Check: 7-year criminal background check within 30 days of start, including SSN trace, counties lived/worked, national criminal and sex offender checks, education verification
- Education Verification: Diploma or transcript acceptable
- Employment Application required
- Work History Verification for last 3 years
- At least 2 recent professional references required
- Various facility-specific documents and acknowledgements must be completed prior to start
This position is ideal for RNs with cardiac rehab or telemetry experience who are comfortable working independently in a busy outpatient setting and have strong EKG monitoring and documentation skills.
