Location: Bend, Oregon
Contract Length: 12 weeks (coverage for caregiver out on approved FMLA)
Requirements
Experience

Competencies
- Medication Test/Pharmacology Competency Exam (as applicable)

Facility Specific Documents (must be completed within 48 hours of signing and no later than 2 Wednesdays prior to start)
- Photo for badge (passport-style with solid background)
- Acceptable Use and Confidentiality Agreement
- Contractual and Third Party Access Agreement
- Compliance and Privacy Program Attestation
- Emergency Contact Form
Health Documents
- Physical Exam (within 1 year of start)
- Tetanus/Diphtheria/Tdap vaccine (within 10 years, given after age 19; declination accepted)
- Negative TB test (PPD, QGold, or TSPOT within 1 year) or Chest X-Ray for positive responders
- TB Questionnaire (updated annually)
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (positive titer required; vaccine series alone not acceptable)
- Varicella
- Drug Testing: Negative 12 Panel Drug Screen within 90 days of start (not required annually)
- Flu Seasonal Vaccination or agency declination/exemption required during flu season (masking required during high transmission)
- COVID Vaccines preferred but not required; vaccine record or declination/exemption required
- Respiratory Safety Clearance (required within 1 year of start and annually)

This position covers a 12-week contract to provide coverage during an approved FMLA absence. All certifications, background checks, health documents, and facility-specific paperwork must be completed and on file prior to start.
