Location: Woodstock, Georgia

Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Average patients per day: 5-8 (assigned 5-10 patients/day equating to roughly 6-8 points).
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Work locations include Assisted Living Facilities (ALFs) and patient homes.
- Must be willing to travel up to 75 miles from home clinic; typical weekly travel about 40 miles.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- On-call requirement: Yes, approximately 1 week per month.
- Charting systems used: HomeCare HomeBase (HCHB), Oasis, Outlook.
- Dress code: Scrubs.
- Contract length: 13 weeks.
- Local candidates accepted at the same rate.
- Contract is a true contract assignment with potential for hire at the end.
Requirements
- Experience: Minimum 1 full year of recent Adult Home Health or Hospice experience (whichever is higher).
- First-timers: Not accepted; must have recent specialty experience.
- Certifications: Current CPR and BLS required.
- Licensure: Must have license in hand; pending licenses not accepted.
- Transportation: Reliable transportation and current driver's license required. Agency-required liability insurance also required.
- EMR Proficiency: Must be proficient with HomeCare HomeBase (HCHB) and Oasis, with usage within the last 6 months preferred. Retraining required if last use was over 6 months ago.
- Weekend and On-call: Varies by location and profession; discuss during interview.
Additional Information
- Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
- Some positions may have a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 Monday-Friday.
Compliance and Documentation Requirements
- Valid Driver’s License/State ID.
- Background checks including 7-year history, SS trace, county, nationwide, sex offender, and state-specific searches.
- Drug screening (10-panel).
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or declination documentation.
- Various state-specific releases and authorizations depending on assignment location.
- Completion of compliance training, confidentiality agreements, HIPAA and privacy policies, and home health/hospice-specific policy reviews and quizzes.
- Proof of liability insurance or personal/auto insurance valid for home health placements.
