Job Details

Experience Requirements

• ENPC – as applicable
• State License Verification (RN)
• NURSYS Verification (RN, LPN/LVN, AP)
• NIH Stroke Certification (annual updates required)
• Pass/Fail Testing for Telemetry RNs (EKG/Medication)
• Medication Test/Pharmacology Competency Exam
• Color Blind Testing (online acceptable)
• Competency Exam (as applicable)
• Orientation Safety Manual Acknowledgement (annual)

Background and Compliance Requirements

• I9 or Attestation

• 7 Year Background Check (updated annually) including:

  • Motor Vehicle Check (Home Health only)

• State-specific checks for Texas (Employability Status, Employee Misconduct Registry) within 30 days of start

Health and Immunization Requirements

• Physical Exam (within 1 year, must be completed within 72 hours of contract signing)
• Fit Mask Test (annual; 3M 1860 Regular or 1860S)
• TDAP Vaccine (within 10 years)
• Negative TB or Chest X-Ray (within 1 year; annual TB questionnaire required)
• Rubeola (Measles) – Positive titer or 2 vaccines required
• Mumps – Positive titer or 2 vaccines required
• Rubella – Positive titer or 2 vaccines required
• Hepatitis B – Positive titer, series, or declination accepted
• Varicella – Positive titer or 2 vaccines required
• Drug Testing – Negative 10-panel drug screen with THC within 10 business days prior to start (no exceptions)
• Flu Seasonal Vaccination or approved exemption (for 2025-26 season)

Additional Requirements

• 2 References within 3 years
• Copy of Resume/Application
• Skills Checklist (annual update)
• Facility-specific documents and acknowledgments to be reviewed and signed prior to start (e.g., vaccination info, TB and occupational exposure, patient-centered pledge, confidentiality, compliance, etc.)
• Orientation modules (~3-4 hours) with parking available at no cost

Notes

• Pass/Fail testing for Telemetry nurses includes 2 attempts (3 with manager approval). No study materials provided; basic Telemetry RN knowledge expected.
• Background checks and health documents must be current and compliant with facility and state requirements.
• Candidates should be prepared for timely completion of all health screenings and documentation within specified timeframes.
