Job Description

• Provide physical therapy services in a home health setting with home visits and periodic office meetings.
• Average patient load: 5-6 patients per day.
• Patient visits include Start of Care and Follow Ups.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Must be willing to travel up to 75 miles from the home clinic.
• Typical schedule: Monday to Friday, 8 AM to 5 PM.
• Weekend rotation: Approximately once a month with a compensatory day off; no overtime.
• On-call: Not required.
• Territory coverage includes Greenwich/Stamford area.
• Expected weekly travel: 200-300 miles.
• Productivity expectations: 30 points per week (points assigned based on visits and activities).
• Electronic Medical Record (EMR) system used: HCHB.
• Locals accepted at the same rate.
• Pending licenses are generally not accepted unless the license is expected within a few days.

Certifications & Licenses Required

• Current Physical Therapy license valid in Connecticut.
• CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
• Must have agency-required liability insurance.
• Must have a current driver's license.

Experience Requirements

• Minimum 1 year of recent Adult Home Health experience.
• First-time home health therapists are not accepted.
• Prior travel experience is not required.

Schedule & Workload

• Standard 40-hour workweek, Monday through Friday.
• Some positions may have a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8s.
• Weekend work approximately once per month.
• No on-call duties.

Travel & Transportation

• Must have reliable transportation.
• Willingness to travel up to 75 miles from the home clinic.
• Mileage reimbursed at IRS standard rate.
• Compensation covers travel from patient home to patient home; commute to/from first and last patient is not billable.

Additional Requirements & Compliance

• Background checks including 7-year history, sex offender search, and state-specific checks as applicable.
• Drug screening (10-panel) required.
• Physical exam within 12 months prior to start.
• Negative TB test or chest X-ray within 12 months.
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• COVID-19 vaccination or declination required.
• Flu vaccination or declination documentation required.
• Education verification and work history verification (at least 2 most recent employers or last 7 years).
• References: Two required.
• Completion of facility-specific compliance training and policy acknowledgments prior to start.
• Must review and sign confidentiality, HIPAA, privacy policies, and other compliance documents.
• Must have personal or auto insurance policy under candidate's name or be listed as a driver.

• Weekend and on-call requirements may vary by location; discuss during interview.
• Contract length is typically 13 weeks.
• EMR experience with HCHB preferred.
• This position is strictly home health; hospice experience is not required.
• Candidates must be prepared to complete all required documentation and compliance training prior to start.
