Competency Exams

• Medication Test/Pharmacology Competency Exam (required for starts on/after 4/14/24, must be completed within 12 months of start)

• Color Blind Testing Online (required for starts on/after 1/16/24, must be completed within 12 months of start)
• Dysrhythmia/EKG Competency Exam (required for EKG Techs, preferred for ER RNs, must be completed within 12 months of start)

Health and Safety Requirements

• Physical Exam: Within 12 months of start, updated annually, signed by PA, NP, DO, or MD
• Fit Mask: Annual fit testing required for all patient-facing candidates; approved mask types specified
• Tetanus/Diphtheria/Tdap: Vaccine within 10 years or signed declination within 12 months of start
• Negative TB Test or Chest X-Ray: Negative QGold or TSPOT within 12 months of start, updated annually; PPDs not accepted after 12/15/2025
• TB Questionnaire: Required if history of positive TB, updated annually
• Immunizations:

  • Rubeola (Measles): 2 MMR vaccines or positive titer (booster if low titer)
  • Mumps: 2 MMR vaccines or positive titer (booster if low titer)
  • Rubella: 2 MMR vaccines or positive titer (booster if low titer)
  • Hepatitis B: Series, titer, or signed declination within 1 year of start
  • Varicella: 2 vaccines or positive titer (booster if low titer)
• Drug Testing: Negative 9-panel drug screen within 90 days of start, updated annually; marijuana not required
• Flu Vaccination: Required during flu season (10/01 - 04/01); vaccine or medical/religious exemption required; declinations not accepted

Background Checks and Verifications

• Background check within 12 months covering all locations lived/worked/schooled in past 7 years, updated annually
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• International background check if candidate has lived/worked internationally in past 7 years
• Illinois Health Care Worker Registry Search required for aides and nursing assistants only (not required for RNs)

Facility-Specific Documents

• Various HIS forms required depending on role (e.g., RN, CNA, Medical Assistant, CST, OR Tech) with legal name and signature requirements
