Registered Nurse - Home Health (Travel/Contract)

Location: Texarkana, Texas
Contract Length: 13 weeks

Job Description

• Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with periodic office meetings.
• Average patients per day: 5-8.
• Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, current driver's license, and agency-required liability insurance.
• Must be willing to travel up to 75 miles from home clinic; estimated weekly travel 100-150 miles.
• Charting required using HomeCare HomeBase (HCHB) and proficiency in OASIS for RNs.
• Schedule: Monday-Friday, 8 AM - 5 PM.
• Weekend rotation: 1-2 times per month.
• On-call requirement: 1-2 weeks per month (varies by location).
• Dress code: Scrubs.
• Local candidates accepted at the same rate.
• Pending licenses NOT accepted; license must be in hand.
• Some positions may have a 4x10 shift schedule (Friday-Monday), but most are standard 5x8.

Requirements

• Specialty: Registered Nurse with Home Health experience.
• Minimum 1 full year of Adult Home Health experience OR 1 full year Hospice experience (whichever applies).
• First-time travelers NOT accepted; must have recent specialty experience.
• Certifications: Current CPR and BLS required.
• Must be proficient with HomeCare HomeBase (HCHB) and OASIS (for RNs).
• Must have reliable transportation and current driver's license.

Productivity Expectations

• Weekly units of productivity: 25-30 units.
• Points system used for visits and activities; assigned 5-10 patients per day equating to roughly 6-8 points daily.

Travel & Location Details

• Territory: Texarkana, Texas area.
• Must be willing to travel up to 75 miles from home clinic.
• Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient not billable).

Compliance & Documentation Requirements

• Must complete standard background checks including:

  • 7-year background check with SS trace, county, nationwide, sex offender, and state-specific searches.
  • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start.
  • Fingerprinting as required by state (varies by location).
• Education verification required.
• Drug testing: 10-panel drug screen required.
• Health documents required:

  • Physical exam within 12 months.
  • Negative TB or chest X-ray within 12 months.
  • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or declination for current season.
  • COVID vaccination series or declination.
• References: Two references required.
• Work history verification: At least two most recent employers or last 7 years if only one employer.

Additional Notes

• Candidates must review and sign various compliance, confidentiality, and policy documents prior to start.
• Candidates must have current liability insurance or be listed as a driver on a policy for auto insurance.
• OASIS E course required if not proficient with HCHB within last 6 months.
• Weekend and on-call requirements vary; discuss during interview.

Prior Travel Experience

• Not explicitly required; however, first-time travelers are NOT accepted. Recent specialty experience is mandatory.
