Job Description

• Provide Physical Therapy services in a Home Health setting.
• Visits include both patient homes and care facilities.
• Average patients per day: 5-6 (assigned 5-10 patients/day).
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have alternate schedules such as 4x10 shifts Friday-Monday).
• Must have reliable transportation and current driver's license.
• Must be willing to travel up to 75 miles from the home clinic.
• Use of EMR systems: HomeCare HomeBase, Oasis, Outlook, Teams.
• Productivity expectations: Minimum of 30 points per week.
• On-call requirements vary by location; discuss during interview.
• Weekend requirements vary; discuss during interview.
• Locals accepted at the same rate.

Requirements

• Experience: Minimum 1 full year of recent Adult Home Health Physical Therapy experience.
• First-timers: Not accepted; must have recent Home Health experience.
• Certifications: Current CPR and BLS certification required.
• Licensure: Must have license in hand (pending licenses accepted only if expected within a few days).
• Prior Travel Experience: Not explicitly required.

• Mileage reimbursed at IRS standard rate; compensated for travel between patient homes (commute to/from first and last patient not billable).
