Job Description
- Work Schedule: Monday, Wednesday, Thursday with call and late days as required
- Specialty Areas: Neuro/Spine, Ortho/Trauma, General/Robotics/Trauma
- Required Experience: Craniotomy and burr hole experience is mandatory

Health & Immunization Requirements
- Physical Exam (within 1 year of start; must be completed within 72 hours of signing contract)
- Fit Mask (3M 1860 Regular or 1860S; annual update required)
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB or Chest X-Ray (within 1 year; time sensitive within 72 hours of contract signing)
- Rubeola (Measles) - Positive titer or 2 vaccines required
- Mumps - Positive titer or 2 vaccines required
- Rubella - Positive titer or 2 vaccines required
- Hepatitis B - Positive titer, series, or declination accepted
- Varicella - Positive titer or 2 vaccines required
- Drug Testing - Negative 10 panel drug screen with THC within 10 business days prior to start; no pending results allowed
- Flu Seasonal Vaccination (required for 2025-26 season; religious or medical exemptions accepted)
Facility Specific Requirements
- Completion and signing of various facility-specific acknowledgments and agreements prior to start (e.g., Vaccination Information, TB and Occupational Exposure, Patient Centered Pledge, Contract Employee Receipt, Computer Security Agreement, Corporate Compliance, Confidentiality Statement, Employee Misconduct Registry, Self Identify Form, Relationship to Physician Form, Authorization for Background Check, Traveler Identification Form)
- Review of Employee Handbook (information only)
- Completion of orientation modules (~3-4 hours)
