Licensure & Verification

• State License: Must be valid in Texas and verified within 30 days of start
• NURSYS Verification: Required within 30 days of start for RN licenses
• Background Checks:

  • 10-year criminal background check within 30 days of start
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
  • Texas DPS Criminal History Search and Employee Misconduct Registry within 30 days of start
• Education Verification: Highest level required, especially for cath lab and CVOR roles

Health & Safety Requirements

• Physical Exam: Required within 1 year of start, must be completed within 72 hours of contract signing
• Fit Mask (N95 Fit Test): Required within 1 year of start; PAPR not accepted
• Immunizations & Titers:

  • TDAP (Tetanus/Diphtheria/Pertussis) within 10 years
  • Negative TB test or chest x-ray within 12 months
  • Rubeola (Measles), Mumps, Rubella: Positive titer or 2 vaccines required
  • Hepatitis B: Positive titer or series (3 doses) required
  • Varicella: Positive titer, 2 vaccines, or documented history of chickenpox required
• Drug Testing: Negative 10-panel drug screen within 30 days of start; additional onsite drug screen during orientation
• Flu Vaccination: Required October 1 - March 31 or agency declination with possible mask requirement

Competency & Documentation

• Competency Exams: Medication test/pharmacology and age-specific competency exams required within 1 year of start
