Job Description

• Acute Inpatient Case Manager role in a large hospital setting
• Must be comfortable managing a caseload of approximately 25 patients
• Responsible for facilitating progression of care, evaluating for the next appropriate level of care, and discharge planning
• Minimum 2 years of experience in a large hospital setting required

Required Certifications & Licenses

• BLS (American Heart Association)
• Current nursing license with Nursys verification within 30 days prior to start
• License/Certificate verification within 30 days prior to start
• Driver’s License/State ID (must be valid and meet address requirements)

Health & Immunization Requirements

• Ascension Color Vision test (completed prior to start, updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; completed within a year of start, updated yearly)
• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
• Fit Test: Required for all travelers, preferred prior to start
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD or IGRA with negative chest X-ray and annual TB questionnaire
• Health Statement or physical exam prior to first assignment, updated annually
• 2 Step PPD or IGRA within required timeframes
• Varicella: Proof of immunity or vaccination (no declinations accepted)
• Tdap: Valid within 10 years, no declinations accepted
• Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually

Professional Requirements

• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding (onsite)
• Ascension Profile (resume or professional profile)
• At least two professional references prior to first assignment
• Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender, and others (completed within 30 days of first assignment, updated annually)
• EPLS check within 90 days of start, rerun if break in service > 90 days
• Ascension Skills Checklist (clinical, completed within one year of start, updated annually)
• Education Verification within 90 days of start, rerun if break in service > 90 days
• Confirmation of Assignment documentation upon initial assignment and each extension
• Employment Verification for the past seven years, updated annually
