Job Opportunity: RN Medical / Surgical / Telemetry

Location: Des Moines, Iowa

Schedule: Every Other Week (EOW)
Contract Length: 13 weeks

Required Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back copies with expiration date.
• ACLS (Advanced Cardiovascular Life Support): Required for RNs in CCU, ED, PACU, MICU, SICU. Scanned copies with expiration date required.
• NIHSS (National Institutes of Health Stroke Scale): Optional for submission; required for onboarding.
• TNCC (Trauma Nursing Core Course): Optional.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required.

Health & Immunization Requirements

• Fit Test: Required prior to onboarding.
• Trinity 10 Panel Drug Screen: Must be completed within 30 days of start (preferably within 14 days). Includes testing for cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, methadone. No THC allowed.
• COVID-19 Vaccine: Proof of September 2025 booster, declination, or exemption (declination not accepted in CA or IL). Valid through October 2026.
• Influenza Vaccine: Proof of vaccine, declination, or exemption (declination not accepted in CA or IL). Valid for flu season 10/1/2024 - 4/30/2025.
• MMR (Measles, Mumps, Rubella): Proof of 2 vaccinations with dates or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
• Varicella: Proof of 1 vaccination with date or IgG titer lab report.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year (for MI, IN, CT, MA) or within 5 years for other states.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard I9 completed by agency.
• Confidentiality & Security Agreement: Signed agreement regarding patient and proprietary information.
• Integrity and Audit Services Form: Signed within days prior to start.
• Non-Employee Upload Form: Completed and uploaded pages 1-3 and 28.
• Specialty Specific Competency Testing: Copy of specialty exam completed within the last year required.
• Criminal Background Check: Federal, nationwide, county criminal check from last 7 years, including SSN verification. Must be dated within 30 days of start.
• Education Verification: Highest level of academic education verified by third party screening.
• EPLS (Excluded Parties List System): Scanned report dated within past 30 days required.
• GSA/OIG Exclusion Check: Scanned report dated within past 30 days required.
• Joint Commission CORE Exam: Completed within the last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start, all aliases checked.
• System Access Form: Must be completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• Conflict of Interest Disclosure Form: Signed form required.
• Child and Adult Dependent Abuse Training: Required for Iowa assignments.
• Iowa Sing Report: Iowa-specific criminal history, sex offender, dependent adult & child abuse registry check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Sanctions Check: Must be completed within 30 days of start.

Additional Notes

• Physical/Health Statement documenting fitness to work within the last year is required.
• Resume, skills checklist (within a year), and two professional references required for submission.

This position is ideal for RNs with Medical/Surgical and Telemetry experience who meet the above credentialing and health requirements and are available for a 13-week long-term assignment in Des Moines, Iowa.
