Required Credentials and Onboarding Requirements

Licenses & Certifications

• BLS (American Heart Association or Red Cross)
• ACLS (American Heart Association or Red Cross)
• NIHSS
• Nursys License Verification
• DCI/BVP (Workplace Violence Prevention training)

Exams & Testing

• Prime Basic Pharmacology Exam (minimum 80%)
• Prime Specialty Exam (minimum 80%)
• Prime Core Competencies Exam (minimum 80%)
• Prime Skills Checklist (submission required)

Professional Requirements

• Resume submission required
• Two professional references required
• Prime Education Verification
• Prime OIG check
• Prime SAM/EPLS exclusion search
• Prime Criminal Background Check (national, within one year)
• Social Security Card or Passport
• Driver’s License, State ID, or Passport
• Prime Badge Photo (clear JPEG headshot)
• Prime Form I-9 (Employment eligibility verification)
• Prime Job Description signed by employee
• Essentials Onboarding Packet completion
• Onboarding Education Attestation (must be completed prior to start)
• Prime Agency Attestation (completed last)

Personal Health History & Immunizations

• COVID-19 Vaccine: 2 vaccines plus booster or declination completed
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Hepatitis B (declinations not accepted at some locations)
• Influenza Vaccine (optional, but recommended during flu season)
• TB Test: Within 1 year of start date
• Annual TB Questionnaire
• Positive PPD with Negative Chest X-Ray (optional)
• Varicella: Vaccination or titer showing immunity
• Tdap (declinations not accepted at some locations)
• Fit Test: Annual respirator fit test required; must be completed prior to start for Illinois locations
• Physical/Health Statement: Must be completed annually and state clearance to work
• Drug Screening: Minimum 10-panel test including THC, within 60 days of start

Additional Notes

• All credentials and onboarding requirements must be completed and verified prior to start.
• Some tests and attestations are site-specific and must be coordinated accordingly.
• Floating between units requires flexibility and adaptability.

This position is suitable for experienced telemetry RNs who meet the above credentialing and health requirements and are comfortable working in a dynamic hospital environment.
