Description
Job Opportunity: RN Cath Lab
Location: Mishawaka, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Work Schedule & Call Requirements
- 1 weekend per month
- Weekday call 4-5 days per pay period (weekend included in total)
- Call hours: End of shift to 7:30 AM Monday-Friday per scheduled call
- Weekend call: End of shift Friday to 7:30 AM Monday
Required Credentials & Certifications
- BLS (Basic Life Support)
- ACLS (Advanced Cardiovascular Life Support)
- NRP (Neonatal Resuscitation Program) – required for Birth Center RNs
- NIHSS (National Institutes of Health Stroke Scale)
- CPI (Crisis Prevention Intervention)
- License/Certificate Verification – must be dated within 30 days of start, including state verification (e.g., NURSYS for RNs/LPNs)
- Trinity Code of Conduct Form – signed form required
- Trinity I9 – standard employment eligibility verification
- Confidentiality & Security Agreement – signed form required
- Integrity and Audit Services Form – signed shortly before start
- Joint Commission Testing (CORE Exam) – within the last year
- Trinity Profile – resume, skills checklist (within a year), and 2 professional references required for submission
- Conflict of Interest Disclosure Form – signed form required
- Criminal Background Check – federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start
- Education Verification – highest academic degree or diploma/GED verified
- EPLS (Excluded Parties List System) Check – dated within 30 days
- GSA/OIG (Office of Inspector General) Check – dated within 30 days
- NSO (National Sex Offender) Check – dated within 30 days, including aliases and state-specific checks if applicable
- System Access Form – must be completed and uploaded one week before start
- Skills Checklist – provided by agency for clinical contractors
- OFAC (Office of Foreign Assets Control) Check – completed within 30 days of start
- Orientation Guide – completion required
Personal Health & Immunization Requirements
- Fit Test
- Annual TB Questionnaire
- Trinity 10 Panel Drug Screen – completed within 30 days of start, must include specific substances, no THC allowed
- Color Vision Test
- Latex Allergy Questionnaire
- COVID-19 Vaccine – proof of booster by Sept 2025, declination or exemption accepted except in CA or IL
- MMR (Measles, Mumps, Rubella) – proof of 2 vaccinations or IgG titer lab report
- Hepatitis B – must have started vaccination series prior to start; titers not accepted
- Influenza Vaccine – proof of vaccine, declination, or exemption for 10/1/2024-4/30/2025 and 10/1/2025-4/30/2026; declination not accepted in CA or IL
- 2 Step PPD OR IGRA (Quantiferon/Tspot) – positive TB requires chest x-ray within last year for Indiana and certain states, otherwise within 5 years
- Varicella – proof of 1 vaccination or IgG titer lab report
- Physical/Health Statement – documentation from exam within last year confirming fitness to work
- Tdap Vaccine – current and non-expired; declinations accepted if no pediatric interaction
Requirements/Qualifications
- Valid RN license in Indiana
- Minimum of 1 year of Cardiac Cath Lab experience
- Cardiac Cath Lab experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
This position requires thorough credentialing and compliance with health and professional standards prior to onboarding.
