Job Opportunity: Physical Therapist (PT) - Home Health

Location: Spartanburg, South Carolina

Job Details

• Specialty: Therapy/Rehabilitation (Physical Therapist - Home Health)
• Coverage Area: 4 counties - Cherokee, Spartanburg, Union, and a small part of Greenville
• Shift: Days, 4 shifts of 10 hours each (Wednesday through Saturday)
• Minimum Units: 7.5 units per shift
• Start Date: ASAP
• On-Call: Not required
• Floating: Yes, within scope to meet facility needs (mileage paid)
• Block Schedule: Not accommodated
• Local Candidates: Not accepted (75-mile radius rule)
• State License: Pending state license accepted but must be obtained prior to starting
• Attendance Policy: More than two missed shifts per 13-week contract without reasonable explanation may lead to contract review and possible cancellation
• Special Requirements: Must have prior PT Home Health experience

Required Certifications & Licenses

• BLS (Basic Life Support): Required (AHA or RQI acceptable)
• State License: Must have valid state license prior to start (Primary Source Verification required)
• Orientation Safety Manual Acknowledgement: Required
• Other Certifications: ACLS, PALS, NRP, TNCC, EFM, NIH Stroke Certification, and others as applicable to the role or unit (check with facility for specifics)

Background & Compliance Requirements

• 7-year criminal background check within 1 year of start, updated annually
• South Carolina statewide search required regardless of residence/work history
• Education verification required (highest level of education must be associates, bachelors, masters, or doctorate; certificates alone are not sufficient)
• E-Verify proof required
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
• Drug testing within 60 days of start (10 panel + Oxycodone + Marijuana)
• Physical exam within 1 year of start, updated annually
• Fit mask (N95) within 1 year, updated annually
• Negative TB test or chest X-ray within 3 months of start
• Immunizations/vaccinations required or approved exemptions for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
  • Seasonal Flu (documentation or declination required during flu season)
  • COVID-19 (Dose 1 and 2 required or declination form)

Additional Requirements

• Must carry government-issued photo ID during hours of service
• References: 2 references within 1 year of start (Qualivis requirement)
• Employment application required
• Skills checklist updated annually for applicable units
• Pass/fail testing (EKG/Medication) required for RNs, not required for Home Health PTs
• Medication test/pharmacology competency exam required within 1 year of start and updated annually

Notes

• Clinician may be required to float within their scope of practice to meet facility needs
• Mileage reimbursement provided for floating
• All requested time off (RTO) must be submitted with initial application; late requests will not be considered
