Registered Nurse - Hospice (Travel/Contract)

Location: Leander, Texas
Contract Length: 13 weeks

Job Summary

Seeking a Hospice Registered Nurse with Homecare Homebase (HCHB) experience who can start immediately. The RN will provide skilled nursing care to patients in their homes, coordinate interdisciplinary care, and develop and oversee plans of care for a geriatric population.

Key Responsibilities

• Provide in-home patient care and interdisciplinary care coordination
• Perform comprehensive patient assessments
• Collaborate with certifying providers to develop and manage Plans of Care
• Complete charting using HCHB and Oasis EMR systems while at patient homes
• Participate in on-call rotation (including one night of call during the week)
• Manage 4-6 patients per day, averaging 30 points per week (40 hours/week)
• Travel up to 50 miles from home base to patient locations

Schedule

• Monday to Friday, 8 AM to 5 PM
• Weekends as needed
• On-call rotation required

Requirements

• Experience:

  • Minimum 2 years overall clinical experience
  • At least 1 year of Home Health experience
• Certifications:

  • Valid RN license in hand (no pending licenses accepted)
  • BLS certification required
• Skills:

  • Proficiency with Homecare Homebase (HCHB) and Oasis charting systems (1 year preferred)
• Other:

  • Non-smoker (due to sensitive patient population)
  • Willingness to participate in on-call rotation
  • Must be able to travel within a 50-mile radius
• First-time travelers accepted

Dress Code

• Scrubs, any color

Compliance and Documentation Requirements

• Current Driver’s License or State ID
• Background checks including 7 years residency/work history, SS trace, county searches, sex offender, FACIS Level 1, Texas state driver’s record
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Certifications: ACLS (AHA or ARC accepted), BLS (required for all units), PALS (AHA or ARC only)
• Medication/Pharmacology competency exam and unit-specific competency exam
• Personal or auto insurance documentation
• Physical exam within 12 months confirming good health
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B (declination accepted), Varicella, and seasonal flu vaccination or declination
• 9-panel drug screen within 12 months
• Professional references within 3 years (2 for travel/allied/per diem acute care, 1 for per diem non-acute care)
• Employment application and annual skills checklist

Additional Notes

• Must be comfortable with on-call duties and weekend coverage as needed
• Mileage reimbursed at IRS standard rates
• Locals within 50-mile radius accepted
