Documentation and Compliance Requirements
- Background check within 30 days of start including:
- Social Security Verification
- National Sex Offender Registry Search
- National Criminal Search
- FACIS L3
- Education verification
- State-specific criminal background checks (NH state police search instead of NH county searches)
- Drug screen: 10-panel within 14 days prior to start
- Physical exam within 1 year prior to start
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within last 10 years)
- Negative TB test or chest X-ray (True 2-Step PPD, Qgold, or Tspot within 1 year)
- Rubeola (Measles), Mumps, Rubella immunity or vaccination
- Hepatitis B immunity or declination waiver
- Varicella immunity or vaccination
- Seasonal flu vaccination (required October 31 - April 1; exemptions allowed with documentation)
- Fit mask test annually (N95 or equivalent)
- Respiratory safety clearance questionnaire required
- Pass/fail Basic Dysrhythmia EKG exam within 48 hours of contract signing (for Southern New Hampshire hospitals)
- Medication and competency exams required internally
References and History
- Two current professional references within the last 3 years (one must be from a supervisor)
- Employment application required
- Skills checklist required
- Internal evaluation required annually for travelers with Aya or agency for 1+ year
This position is suitable for healthcare professionals with at least 2 years of experience in Radiology/Cardiology procedures, who hold the required certifications and licenses, and are comfortable with call responsibilities and working in a fast-paced cardiac catheterization lab environment. Prior travel experience is not mandatory.
