• 100 bed facility (96 semi-private, 4 private)
• 2 floors: LTC and LTC-SNF
• Dementia and Alzheimer's patients with secure care tracker system

Patient Population / Common Diagnoses

• Diabetes Mellitus (DM)
• Chronic Obstructive Pulmonary Disease (COPD)
• Post Stroke
• Heart Disease
• Fractures
• Dementia and Alzheimer's
• Rehab patients
• Chronic conditions with multiple comorbidities

Job Duties

• Medication administration and passes
• Vital signs monitoring
• Blood sugar checks and management
• Wound care
• Ostomy management
• Foley catheter management
• Tube feeding administration
• Assisting other team members
• Occasional IV antibiotics and potential PICC line management

Work Environment

• Nurse to patient ratio: 1:17-20
• No tele monitors
• Respiratory treatments managed by RNs and LPNs (CPAP, nasal cannula, high flow; no trachs or vents)
• No Respiratory Therapist on-site 24/7
• Pharmacy: Bubble packs and house stock of generic meds

Team and Supervision

• Interdisciplinary team includes PT, OT, SLP, Case Managers, Social Workers, Dieticians
• Supervisor available on every shift
• Physician coverage includes NP rounds M-F mornings, MD coverage on select evenings and bi-monthly visits
• Daily team huddles required

Shifts & Scheduling

• Days: 7 AM - 3 PM or 7 AM - 7 PM
• Evenings: 3 PM - 11 PM
• Nights: 11 PM - 7 AM or 7 PM - 7 AM
• Schedule made 6 weeks in advance
• Travelers expected to be flexible to fill staffing gaps
• Weekend requirement: every other weekend
• Holiday work may be required
• No call or standby requirements
• Floating between floors required

Orientation

• 3 shifts with a preceptor

Dress Code

• Any color scrubs, professional appearance
• No large earrings/jewelry, no fake nails
• Hair must be up
• No face tattoos
