Key Responsibilities

• Review patient records to determine appropriateness of medication therapy
• Evaluate patient conditions to ensure all issues are addressed
• Develop medication plans minimizing risk of adverse side effects
• Consult with prescribers, nurses, and patients regarding medications and dosages
• Advise on correct drug administration
• Assess outcomes of pharmaceutical treatments
• Collaborate with healthcare professionals to optimize patient care
• Maintain accurate documentation of medication plans and patient progress
• Routinely consult with physicians and nurses on medication orders, availability, side effects, incompatibilities, formulary considerations, and care path management
• Participate in education of students and residents as needed
