Job Opportunity: Registered Nurse Case Manager / Primary Care Clinic Nurse
Location: Juneau, Alaska
Employment Type: Contract
Contract Length: 13 weeks
Job Summary
This is a hybrid/blended nursing role combining case management and clinic nursing responsibilities. The nurse will actively manage assigned patients and serve as a clinic nurse, collaborating with medical providers and interdisciplinary teams to optimize patient outcomes.
Key Responsibilities
- Manage acute, chronic, and health maintenance care needs through continuous review of EHR reminders and patient outreach.
- Coordinate multi-disciplinary care conferences for high-risk or complex patients.
- Participate in case management reviews with a multi-disciplinary team.
- Advocate for patients and families, promoting decision-making, independence, and self-care.
- Facilitate and track patient referrals within and outside the organization, ensuring timely follow-up and resolution of obstacles.
- Perform staff nursing duties including assessment, documentation, patient education, and medication/therapy administration.
- Maintain annual competencies related to safety and infection control.
- Participate in triage and respond to emergency situations as an RN.
- Collaborate effectively with team members across departments and contribute to clinic improvement efforts.
Required Qualifications
- Active, unrestricted Registered Nursing license in Alaska at time of application.
- Graduate of an accredited nursing school (Bachelor’s degree preferred).
- Basic Life Support (BLS) certification.
- At least 2 years of case management experience within the last 2 years (higher experience requirement interpreted from description).
- Proof of flu vaccine required.
- Fingerprint cards required for onboarding (start date minimum 5 weeks after receipt).
Preferred Qualifications
- National Case Management certification.
Skills and Knowledge
- Knowledge of nursing process and ability to apply it in clinical settings.
- Familiarity with tribal health services in Alaska preferred.
- Strong customer service, safety, and infection control principles.
- Proficient in assessment, data collection, and use of medical/computer equipment.
- Excellent oral and written communication skills.
- Ability to multi-task, work independently, and problem-solve in a fast-paced environment.
- Physical ability to stand for long periods, bend, stoop, climb, and move quickly as needed.

- Must live at least 50 miles away from the facility to be eligible.
