Job Opportunity: RN Medical / Surgical / Telemetry

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Every Other Week (EOW)

Required Specialty

• Registered Nurse (RN) with experience in Medical/Surgical and Telemetry units
• Years of Experience: Not explicitly stated; assume standard RN experience for specialty

Certifications & Licenses Required

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel
• ACLS (Advanced Cardiovascular Life Support): Required for RNs working in CCU, ED, PACU, MICU, SICU
• NIHSS (National Institutes of Health Stroke Scale): Optional but preferred
• TNCC (Trauma Nursing Core Course): Optional
• Valid RN License: Must be current and verified within 30 days of start date

Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start, no THC)
• COVID-19 Vaccine (including booster by Sept 2025) or approved exemption/declination (declination not accepted in CA or IL)
• MMR (Measles, Mumps, Rubella): Proof of 2 vaccinations or IgG titer
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine: Proof required for current flu season or approved exemption/declination (declination not accepted in CA or IL)
• 2 Step PPD or IGRA (Quantiferon/Tspot) for TB screening
• Varicella: Proof of vaccination or IgG titer
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Tdap Vaccine: Current and non-expired; declinations accepted if no pediatric interaction

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (completed)
• Specialty Specific Competency Testing (copy of exam within last year)
• Trinity Profile: Resume, Skills Checklist (within last year), 2 professional references
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check: Federal, Nationwide, County (last 7 years), SSN verification, employment verification
• Education Verification: Highest academic degree or diploma verified
• EPLS (Excluded Parties List System) Check
• GSA/OIG (Office of Inspector General) Check
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) Check (within 30 days of start)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check (within 30 days prior to start)
