Job Description

• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
• Tdap: Valid within 10 years, no declinations accepted
• TB Testing: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
• Fit Test: Required for all travelers (preferred prior to start, cannot be completed onsite)
• Health Statement: Completed physical exam or statement of good health prior to first assignment, updated annually
• Color Vision Test: Completed prior to start and updated every 3 years
• 12 Panel Drug Screen: Completed within 30 days prior to start, updated annually

Required Professional Documentation & Checks

• Resume/Profile submission
• At least two professional references prior to first assignment
• Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (completed within 30 days prior to first assignment)
• EPLS (Excluded Parties List System) check within 90 days prior to start
• Education Verification relevant to clinical work (within 90 days prior to start)
• Employment Verification for past 7 years
• Confirmation of Assignment documentation
• Ascension-specific onboarding requirements and attestations

Required Exams & Checklists

• Ascension Cerner Test-Out (Checklist/Testing)
• Ascension Skills Checklist (Clinical competency, updated annually)
• Ascension Pharmacology Exam (minimum 80% score, updated every 3 years)
• Ascension Specialty Exam (minimum 80% score, updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
• Ascension EKG Exam (minimum 80% score, updated as required)
