• Ascension Nova StatStrip pre-hire modules
• Ascension Cerner Test-Out (clinical system competency)
• Health Statement or physical exam prior to first assignment, updated annually
• Ascension Skills Checklist (clinical skills, updated annually)
• Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
• Ascension Criminal Background Check (including Social Security Trace, OIG Search, National Sex Offender, updated annually)
• Ascension EPLS (Exclusionary List) check within 90 days prior to start
• Ascension Education Verification (within 90 days prior to start)
• Ascension Employment Verification (past 7 years, updated annually)
• Ascension Confirmation of Assignment (provided upon initial assignment and updates)

Immunizations & Health Requirements

• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire (updated annually)
• 2 Step PPD or IGRA test within 1 year prior to start
• Varicella: Proof of immunity or two vaccine doses (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• Fit Test: Required annually (preferred prior to start, can be completed onsite)

Additional Notes

• Nursys verification and disciplinary action disclosures required prior to start
• Resume/Profile and at least two professional references required prior to first assignment
• Some optional checklists and evaluations may be requested but are not mandatory for onboarding
