Job Description

• Provide routine daily nursing care and other services according to each resident’s assessment, choices, and care plan.
• Collaborate closely with the RN Manager to manage day-to-day functions.
• Facility size: 160 beds.
• Resident-to-nurse ratios to be discussed during the interview.
• Must have BLS and ACLS certifications.
• Position can also be filled by an LPN.

Required Experience

• Specialty: RN LTC

Certifications & Licenses Required

• BLS (Basic Life Support) – Required for clinical positions.

• Health statement/physician statement within last 12 months.
• Unexpired government-issued photo ID.
• Varicella: 2 vaccines or positive titer; booster or declination accepted.
• Tdap vaccine valid within 10 years; declination accepted.
• Background check within 30 days of start (includes SS trace, county searches, nationwide sex offender check).
• Iowa Adult and Child Abuse clearance.
• EPLS/GSA/SAM HHS/OIG check within 30 days of start.
• Professional references: 2 required, including 1 supervisor reference.
• Resume or profile submission required.
• I-9 attestation completed.
• Color vision test.
• OSHA questionnaire completed by candidate.
• Influenza vaccine or facility declination accepted (seasonal requirement, due by Dec 1 annually).
