Job Opportunity: RN Step Down (PCU)

Location: Lafayette, Louisiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Unit Details

  • 28 beds in the Step Down/Progressive Care Unit (PCU)

Required Specialty Experience

  • Registered Nurse (RN) with experience in Step Down/PCU

Health & Immunization Requirements

  • MMR (Measles, Mumps, Rubella) – Proof of 2 immunizations, positive titer, or declination with signed statement
  • Hepatitis B – Proof of 3 immunizations, positive titer, or declination with required signatures
  • Influenza Vaccine – Required during flu season (10/1/2025 - 4/30/2026); medical or religious exemptions accepted with proper documentation
  • TB Test – Negative QuantiFERON blood test within 1 year or 2-step TB skin test; if positive, must provide clearance or treatment proof
  • Varicella – Proof of 2 immunizations, positive titer, or declination with signed statement
  • Tdap – Proof of vaccination within past 10 years or declination with required signatures
  • Annual TB Questionnaire – Required within 6 months of start with documentation if positive

Additional Health & Safety Requirements

  • Fit Test for N95 mask (3M 1860, 1860s, or 1870+) completed within 1 year prior to start and updated annually
  • Health Statement signed and dated within 1 year stating ability to work without restrictions
  • FMOLHS Eye Exam & Color Vision Test (Ishihara Color Test and Snellen Vision) within 1 year prior to start
  • Negative 10 Panel Drug Screen within 30 days prior to start (includes marijuana/THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)

Professional & Background Requirements

  • FMOLHS Resume submission required
  • FMOLHS Criminal Background Check within 30 days prior to start including:
    • SSN Address Trace (7 years)
    • County Criminal Searches (7 years)
    • National Criminal Check including NSO, OFAC, OIG, SAM
    • Education Verification (3rd party)
    • Employment Verification (last 5 years)
    • Driving Record (if applicable)
  • FMOLHS Skills Checklist – Unit specific, updated annually, within 1 year prior to start
  • FMOLHS Competency Exam – Passing score of 80% or higher within 1 year prior to start
  • FMOLHS Core Competencies – Includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; updated annually with passing score of 80% or higher
