Job Opportunity: OT Outpatient - Therapy/Rehabilitation

Position Details

• Specialty: Occupational Therapy (OT) Outpatient with Lymphedema experience required
• Shift: 5x8 hour days (8:00 AM - 4:30 PM), open to 4x10 hour days
• Weekend Requirement: Rotating every 6 weeks
• On-call: Not required
• Floating: May float within scope to meet facility needs
• Block Schedule: Possible, to be discussed during interview
• Locals Accepted: Yes, no radius restrictions
• Return to Office Restrictions: Ideally no more than 5 days

Experience & Certification Requirements

• Minimum Specialty Experience: 1 year (OT with Lymphedema experience)
• Prior Travel Experience: Not explicitly required
• Certifications:

  • BLS (Basic Life Support) required
  • State OT license required
  • NNBCOT certification required

Compliance & Documentation Requirements

• Valid/current US Driver’s License or US ID
• Background Checks covering all locations lived/worked in past 7 years, including:

  • SSN Trace, Counties, States, National Criminal, National Sex Offender, FACIS Level 3
  • EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
• Fingerprinting: Illinois Health Care Worker Registry Search required for clinical candidates without IL state license; must be completed in Illinois 3 business days prior to start
• Education Verification: Initial degree verification required
• Competency Exam: Required for all clinical candidates
• Photo for Badge: Required within one week of signing contract
• Code of Conduct: Must review and sign within 7 days of contract signing
• Physical Exam: Required within one year of start, must confirm good physical and mental health, free of communicable disease, and ability to perform job functions without restrictions
• Fit Mask Test: Required for new contracts (specific mask types accepted)
• Negative TB or Chest X-Ray: Required within one year of start
• TB Questionnaire: Required annually
• Immunizations or documentation required within 7 days of contract signing:

  • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines, positive titer, or signed declination)
  • Hepatitis B (vaccine series, positive titer, or signed declination)
  • Varicella (2 vaccines, positive titer, or signed declination)
• Drug Testing: 9-panel drug screen within 30 days of start
• Flu Vaccination or Medical/Religious Exemption: Required annually by 11/30
