Location: Decatur, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks
Position Details
- Specialty: Occupational Therapy (OT) Outpatient with Lymphedema experience required
- Minimum Years of Experience: 1 year
- First-time travelers accepted
- Shift Options:
- 4x10 hour days (8:00 AM - 4:30 PM)
- Open to 5x8 hour days as well
- Weekend Requirement: Rotating every 6 weeks
- On-call: Not required
- Floating: May float within scope to meet facility needs
- Block Schedule: Possible, to be discussed during interview
- Locals accepted with no radius restrictions
- Return to Office (RTO) Restrictions: Ideally no more than 5 days

Required Certifications & Licenses
- BLS (Basic Life Support) - AHA (RQI acceptable)
- State License (Illinois)
- NNBCOT certification required
- ACLS, PALS may be required based on unit/specialty
- Primary source verification required for all certifications and licenses
Experience Requirements
- Minimum 1 year of experience in OT Outpatient with Lymphedema
- Prior travel experience: Not explicitly required
Background & Compliance Requirements
- Background Check within 30 days of start covering all locations lived/worked in past 7 years
- EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
- Fingerprinting: Illinois Healthcare Worker Registry Search required for clinical candidates without IL state license; must be completed in Illinois 3 business days prior to start
- Education Verification: Initial degree verification required
- Competency Exam: Required for all clinical and allied candidates
- References: 2 professional references required for direct travel allied candidates
Health & Safety Requirements
- Physical Exam: Within 1 year of start, must indicate good physical and mental health, free of communicable disease, and ability to perform job functions without restrictions
- Fit Mask Test: Required for new contracts (specific mask types accepted)
- Negative TB Test or Chest X-Ray: Within 1 year of start
- TB Questionnaire: Required annually
- Immunizations:
- Measles (Rubeola), Mumps, Rubella (2 MMR vaccines, positive titer, or signed declination)
- Hepatitis B vaccine series, positive titer, or signed declination
- Varicella (2 vaccines, positive titer, or signed declination)
- Drug Testing: 9-panel drug screen within 30 days of start
- Flu Vaccination or Medical/Religious Exemption required annually by 11/30
Additional Notes
- Guaranteed hours with some allowance for client call-offs
- Badge photo required within one week of signing contract
- Various state-specific criminal and offender registry searches required within 30 days of start
- Parking permit form and code of conduct acknowledgment required within 7 days of contract signing
