Position Details

Background & Health Requirements

  • Background Checks: 7-year criminal background check, EPLS/GSA/SAM, HHS/OIG checks required within 30 days of start
  • Physical Exam: Required within 12 months of start
  • Drug Testing: 10-panel drug screen required within 30 days of start (no rapid screens)
  • Immunizations & Health Documentation:
    • Tetanus/Diphtheria/Tdap required
    • Negative TB test (TSpot or QGold) within 12 months; annual updates required
    • Measles, Mumps, Rubella (MMR) immunity or vaccination documentation required
    • Varicella immunity or vaccination documentation required
    • Hepatitis B vaccination series and/or positive titer or signed declination form required
    • Seasonal Flu Vaccination or approved medical/religious exemption required during flu season (Oct 1 - Mar 31)
  • Fit Mask Test: Completed onsite during orientation; updated annually

Documentation & Other Requirements

  • Valid government-issued photo ID or passport (name must match all documentation)
  • I9 or attestation required
  • E-Verify or attestation required
  • Photo for badge (passport style, clear background)
  • Signed job description prior to start
  • Completion of facility-specific forms including security questions and vaccine exemption forms if applicable
  • Two professional references within 3 years of hire required
