Job Opportunity: Occupational Therapist (OT) - Full Time Traveler (Days)

Location: Pampa, Texas
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Requirements

• Bachelor’s or Master’s Degree in Occupational Therapy
• Current state Occupational Therapy certification/license
• Current BCLS (AHA) certificate
• National Board for Certification in Occupational Therapy preferred
• Membership in AOTA and OTAC preferred
• Minimum 2 years of experience in Occupational Therapy (higher value taken from 1-2 years stated)

Certifications and Compliance Requirements

• BLS (American Heart Association or Red Cross)
• COVID-19 Vaccine: 2 vaccines and booster required or declination completed (optional)
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Hepatitis B vaccination (declinations not accepted)
• Influenza Vaccine or declination required (Nov 1, 2025 - Mar 31, 2026)
• TB Test within 1 year of start date
• Varicella vaccination or titer showing immunity
• Tdap vaccine (declination only for medical reasons with PCP note)
• Positive PPD with negative chest X-ray proof (optional)
• Annual Physical/Health Statement stating clearance to work
• Driver’s License, State ID, or Passport required

Professional and Onboarding Requirements

• Prime OIG check
• Prime Core Competencies Exam (must score 80% or higher)
• Prime Skills Checklist
• Prime Color-blind test (annual)
• National Criminal Background Check within 1 year of start date (some state-specific timing requirements)
• Prime Agency Attestation (completed after all credentials uploaded)
• Prime SAM/EPLS exclusion search
• Prime Form I-9 (Employment eligibility verification)
• Prime Specialty Exam (must score 80% or higher)
• Prime Orientation Packet (includes Essentials Form and Post Test)
• Signed Job Description
• Education Verification (Diploma or Transcripts accepted)
• DCI/BVP training for Workplace Violence Prevention
• Drug Screening (minimum 10-panel test including THC) within 60 days of start (CA locations have specific testing requirements)
