Job Opportunity: Medical Technologist

Location: Silvis, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 26 weeks (approximately 6 months)

Work Schedule

• 9:00 AM – 5:30 PM
• No weekends

Required Credentials & Certifications

• ASCP certification required
• BLS (American Heart Association) certification required for all direct patient care personnel
• Current license/certification verification dated within 30 days of start
• Fit Test
• Trinity Specialty Specific Competency Testing (specialty exam completed within the last year)
• COVID-19 Vaccine: Proof of booster by Sept 2025, declination or exemption not accepted in Illinois
• MMR: Proof of 2 vaccinations or IgG titer
• Hepatitis B: Must have started vaccination series prior to start (titers not accepted)
• Influenza Vaccine: Proof of vaccine or declination (declination not accepted in Illinois) for 2024-2025 and 2025-2026 seasons
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella: Proof of vaccination or IgG titer
• Tdap: Current, non-expired vaccine; declinations accepted if no pediatric interaction
• Physical/Health Statement: Exam within the last year confirming fitness to work

Professional & Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 form completed by agency
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28)
• Trinity Profile: Resume, Skills Checklist (within a year), 2 professional references
• Child and Adult Dependent Abuse Training (Iowa only)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check: Federal, Nationwide, County (last 7 years), SSN verification, dated within 30 days of start
• Education Verification: Highest academic degree or diploma/GED verified
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• Iowa-specific Criminal History and Abuse Registries check (if applicable)
• Joint Commission CORE Exam within the last year
• OFAC sanctions check within 30 days
• NSO (National Sex Offender) check dated within 30 days (Michigan includes statewide check)
• System Access Form completed and uploaded one week before start
• Skills Checklist provided by agency for clinical contractors
