Job Opportunity: Registered Nurse Case Manager
Location: Juneau, Alaska
Employment Type: Contract
Contract Length: 13 weeks
Key Requirements
- Licensure: Must have an active, full, unrestricted Alaska RN license at time of application.
- Experience:
- Minimum 2 years of recent case management experience within the last 4 years.
- At least 1 year of general nursing experience, including:
- 6 months as a clinical nurse in an outpatient clinic, OR
- 2 years general nursing with at least 1 year in ambulatory care or case management.
- Certifications:
- Basic Life Support (BLS) preferred.
- National Case Management certification preferred.
- Vaccinations: Proof of flu vaccine required.
- Additional: Must live at least 50+ miles away from the work location.
Job Responsibilities
- Actively manage patient care in collaboration with medical providers and interdisciplinary teams to maximize healthcare outcomes.
- Coordinate multi-disciplinary care conferences for high-risk or complex patients.
- Manage acute, chronic, and health maintenance care needs through EHR review, patient outreach, and education.
- Participate in case management reviews with multi-disciplinary teams.
- Advocate for patients and families, promoting decision-making, independence, and self-care.
- Facilitate and track patient referrals within and outside the organization, ensuring timely follow-up and resolution of obstacles.
- Perform nursing duties including assessment, documentation, education, and medication/therapy administration.
- Maintain annual competencies including safety and infection control.
- Collaborate effectively with team members across departments and participate in team improvement efforts.
- Serve as a leader among nursing staff and unit coordinators.
- Participate in triage and emergency response as an RN.
- Support specialty clinics by managing referrals, scheduling, and assisting providers during procedures.
- Manage OB case management including patient intake, education, outreach, and postpartum/newborn care tracking.
- Coordinate video teleconferencing visits with specialty providers.
- Manage referrals and appointments for Exercise Treadmill Tests (ETT).
Skills and Abilities
- Strong assessment, data collection, and problem-solving skills.
- Ability to multi-task and work independently in a fast-paced clinical environment.
- Effective oral and written communication skills.
- Ability to maintain compliance with annual competencies and infection control policies.
- Physical ability to stand for long periods, bend, stoop, climb, and move quickly as needed.
- Ability to follow complex directions and perform multiple tasks simultaneously.
Additional Information
- No housing or travel accommodations provided.
- Must be able to submit fingerprints and complete processing prior to start date (start date typically 5 weeks after fingerprint submission).
