Location: Springfield, Massachusetts

Work Schedule Options

40 hours/week:
- Sunday: 7:00 AM - 5:30 PM
- Monday: 7:00 AM - 5:30 PM
- Thursday: 10:30 AM - 9:00 PM
- Friday: 7:00 AM - 5:30 PM
30-32 hours/week (alternative):
- Sunday: 6:00 AM - 6:30 PM
- Monday: 6:00 AM - 5:30 or 6:30 PM
- Friday: 7:00 AM - 5:30 PM

Required Certifications

ACLS (Advanced Cardiac Life Support) from American Heart Association or American Red Cross
BLS (Basic Life Support), preferably from American Heart Association or American Red Cross
PALS (Pediatric Advanced Life Support), preferably from American Heart Association or American Red Cross
National Tech Certification as applicable
State License verification (primary source online verification)
NURSYS Verification (for RN, LPN/LVN, AP)

Required Documentation & Compliance

Government issued photo ID (Driver’s License/State ID)
Emergency Contact Form (prior to start)
Background Checks (7-year search, EPLS/GSA/SAM, HHS/OIG) updated annually
Medication Test/Pharmacology Competency Exam (as applicable)
Competency Exam (as applicable)
Clear passport-style photo for badge
Facility-specific forms including flu medical/religious exemption, TB risk assessment, COVID & flu exemption, Hep B & TDAP declination, demographics, emergency contact, code of conduct, confidentiality agreement, and vaccine declination forms as applicable
Physical exam within 1 year of start
Respirator Fit Test (N95) within last 12 months including OSHA Respiratory Form
Negative TB test or chest X-ray within 1 year of start (with specific rules for past positives)
Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccines, or history of chickenpox)
Drug Testing: 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
Seasonal Flu Vaccination or Declination during flu season (10/1 - 3/31)
COVID-19 Vaccination proof or declination (with specific rules for vaccinations before/after 9/12/23)
Respiratory Safety Clearance including completed OSHA questionnaire

References & Experience

2 Professional References required for Travel, Allied, Per Diem Acute Care
1 Professional Reference required for Per Diem Non-Acute Care
References not required for Non-Clinical roles
References must be within 3 years of start
