Home Care RN - Vermont (St Albans)

Job Overview
Provide comprehensive home care nursing services with a focus on hospice coverage when on-call. Serve as a case manager for assigned patients, ensuring quality care through assessment, planning, implementation, and evaluation in accordance with evidence-based practices and regulatory standards.
Key Responsibilities
- Complete OASIS documentation accurately and timely, staying current with documentation requirements.
- Assess patient and caregiver needs using the nursing process to deliver quality care.
- Develop and communicate realistic, evidence-based plans of care involving patients and caregivers.
- Manage caseloads and visit times to meet agency productivity standards.
- Implement medical plans of care, nursing interventions, and procedures ensuring patient safety and quality.
- Maintain proficiency in high-tech nursing skills such as IV use, wound VAC, and complex wound care.
- Collaborate with LPNs within their scope of practice, delegate tasks, and review documentation.
- Evaluate patient responses and care effectiveness to ensure quality outcomes.
- Maintain accurate and up-to-date nursing documentation for effective healthcare team communication.
- Follow agency protocols for visit documentation and completion.
- Educate patients and caregivers on medication side effects, treatment options, discharge instructions, and reportable conditions.
- Identify and document interdisciplinary patient needs, coordinating with physical therapy, social work, and other disciplines.
- Complete required agency, state, and federal forms timely (e.g., HHABN, Medicare Notice of Non-Coverage).
- Conduct visits efficiently, ensuring supervisory visits are not combined with routine visits.
- Assess needs for Home Care Aide (HCA) services, develop and coordinate plans of care, and provide ongoing supervision.
- Report patient incidents promptly to Quality Improvement specialists.
- Coordinate safe patient discharges and transfers, ensuring proper documentation and compliance.
- Collaborate with physicians, providers, team members, and vendors to support patient care plans.
- Participate in monthly peer review audits, providing and accepting constructive feedback professionally.
- Schedule and facilitate case conferences to enhance team communication and documentation.
- Advocate proactively for patient care to ensure sensitivity to individual patient and family needs.
Requirements
- Licensed Registered Nurse (RN) with adherence to Vermont State Board of Nursing regulations.
- Experience in home care nursing with knowledge of state and federal regulations.
- Ability to perform high-tech nursing skills (IV therapy, wound care).
- Strong documentation and case management skills.
- Effective communication and interdisciplinary collaboration abilities.
- Hospice coverage experience required when on-call.
