Job Description

• Position: Clinical Pharmacist
• Schedule: 40 hours per week with rotating on-call duties
• On-call responsibilities limited to critical emergencies or variable stability situations
• Licensure: Nebraska (NE) license required (NE license process takes approximately 3 months)
• Open to permanent, temp-to-perm, and travelers
• Urgent need to fill the position

Required Qualifications

• Valid Nebraska Pharmacist License
• ACLS (Advanced Cardiac Life Support) certification from American Heart Association or American Red Cross
• BLS (Basic Life Support) certification, preferably from American Heart Association or American Red Cross
• PALS (Pediatric Advanced Life Support) certification preferred
• Medication Test/Pharmacology Competency Exam required
• Physical exam within 1 year of start
• Negative TB test or chest X-ray within 1 year of start (with specific guidelines for past positives)
• Immunizations or proof of immunity for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9 Panel Drug Screen within 1 year of start
• Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)
• Background checks including SS Trace, County searches, Nationwide, Sex Offender, EPLS/GSA/SAM, and HHS/OIG (updated annually)
• Government issued photo ID required
• Emergency Contact Form required prior to start
• Professional references: 2 for travel, allied, per diem acute care; 1 for per diem non-acute care (within 3 years)
• Annual skills checklist required

Experience
