Job Opportunity: Outpatient Physical Therapist (PT) - Travel/Contract

Location: Seaside, Oregon

  • Floating: No floating between units
  • On-call: Not required

Required Qualifications

  • License: Valid Physical Therapist license for Oregon
  • Experience: Minimum 1 year outpatient physical therapy experience required
  • Prior Travel Experience: Not explicitly required
  • Certifications:
    • BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation certification
    • ACLS (Advanced Cardiac Life Support) - AHA or Canadian Heart & Stroke Foundation certification (verify per clinical track)
    • Other certifications (PALS, NRP, Chemotherapy, EFM, NIH Stroke, CPI, IV Certification, MAB) as applicable per clinical track and specialty requirements
  • Background Checks:
    • Comprehensive background check within 45 days of start, covering all locations lived/worked in past 7 years
    • State-specific searches as required (Oregon Abuse/Neglect, Washington WSP/WATCH for border placements)
    • Education verification required
  • Health Requirements:
    • Physical exam within 1 year of start
    • Immunizations: TDAP, MMR (Rubeola, Mumps, Rubella), Varicella, Hepatitis B (or declination), Flu seasonal vaccination (or declination), COVID vaccine or declination
    • Negative TB test or chest X-ray within 12 months
    • Drug screening: 10 panel within 90 days of start (no instant tests)
    • Respirator clearance (RespSafety) for travel candidates starting on or after 2/5/2024
    • Fit mask testing (annual) for travel candidates starting on or after 2/5/2024
  • References: Minimum 2 supervisor references covering 1 year of employment within last 3 years in a comparable specialty/setting
  • Employment Application: Must include at least last 5 years of employment with month/year format and explanation for gaps/overlaps
  • Competencies: Medication test/pharmacology competency exam as applicable
  • Skills Checklist: Required for applicable units, updated annually

Additional Requirements

  • Training Modules:
    • Completion of Providence WorkRamp Core and Specialty modules as assigned based on region, profession, and specialty
    • Epic training modules required within a week of orientation
    • Annual regulatory and specialty-specific modules required for travel and per diem caregivers in ER, L&D, leadership, hospice/home health, and assisted living settings
  • Facility Policies:
    • Float policy acknowledgement (may be required to float to other Providence facilities within reasonable distance)
  • Photo for Badge: Passport-style photo with neutral background required

This is a travel contract opportunity for an experienced outpatient physical therapist in a critical access hospital setting in Oregon. The position requires outpatient PT experience, appropriate licensure, and completion of required certifications, background checks, health screenings, and training modules. The role does not require floating, on-call, or weekend shifts, and offers a patient ratio of approximately 1:11.
