Required Qualifications

Background and Health Requirements

• 7-year background check including county criminal background, sex offender search, and state police investigation (behavioral health units only)

• Physical exam within 12 months of start
• Negative TB test (TSpot or QGold) within 12 months; chest X-ray if positive history
• Fit mask test completed onsite during orientation, updated annually
• Drug testing: 10-panel required within 30 days of start (no rapid screens)
• Immunizations and Titers:

  • Tdap required for all travelers
  • Hepatitis B: Vaccine series and positive titer or signed facility-specific declination form
  • MMR (Measles, Mumps, Rubella): Positive titers or documentation of 2 vaccines; boosters if titers low/equivocal
  • Varicella: Positive titers or documentation of 2 vaccines; boosters if titers low/equivocal
  • Seasonal Flu Vaccination or approved medical/religious exemption required during flu season (Oct 1 - Mar 31)

Additional Requirements

• I9 or attestation required
• Valid government-issued photo ID (passport acceptable in lieu of driver's license or state ID)
• Photo for badge (passport style, clear background)
• Completion of facility-specific forms including job description acknowledgment, NESF form, and vaccine exemption forms if applicable
• Two professional references within 3 years of hire
