Job Opportunity: Registered Nurse (RN) - Medical/Surgical/Telemetry

Location: Dubuque, Iowa
Employment Type: Long-Term Assignment (LTA)

Job Description

• Every Other Week (EOW) and Holiday rotation (holidays included in contract such as Easter, Mother’s Day, Father’s Day, Memorial Day)

Required Qualifications

• License: Valid RN license for Iowa

• Certifications:

  • BLS (American Heart Association) – required for all direct patient care personnel
  • ACLS (American Heart Association) – required for RNs in critical care areas
  • NIHSS certification

• Health & Safety:

  • Fit Test
  • Trinity 10 Panel Drug Screen (completed within 30 days of start)
  • COVID-19 Vaccine (proof of booster, declination or exemption accepted except in CA or IL)
  • MMR vaccination or immunity proof
  • Hepatitis B vaccination series (titers not accepted)
  • Influenza vaccine or declination (dates apply)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive
  • Varicella vaccination or immunity proof
  • Tdap vaccine (declinations accepted if no pediatric interaction)
  • Physical/Health Statement confirming fitness to work (within last year)

• Professional Requirements:

  • License/Certificate Verification (dated within 30 days of start)
  • Trinity Code of Conduct Form
  • Trinity I9 Form
  • Trinity Confidentiality and Security Agreement
  • Trinity Integrity and Audit Services Form

  • Trinity Specialty Specific Competency Testing (attach specialty exam completed within a year)
  • Trinity Profile (resume, skills checklist within a year, 2 professional references)
  • Trinity Conflict of Interest Disclosure Form
  • Criminal Background Check (Federal, Nationwide, County for last 7 years, SSN verification, dated within 30 days)
  • Education Verification (highest academic degree or diploma/GED)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG Exclusion check dated within 30 days
  • Trinity Joint Commission Testing (CORE Exam within a year)
  • NSO (National Sex Offender) check dated within 30 days
  • Trinity System Access Form (must be completed and uploaded one week before start)
  • Trinity Skills Checklist (provided by agency for clinical contractors)
  • Child and Adult Dependent Abuse Training (Iowa only)
  • Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
  • OFAC Sanctions Search (within 30 days of start)
