Job Opportunity: OT Home Health (Travel/Contract)
Location: Portland, Oregon
Specialty: Therapy/Rehabilitation (Occupational Therapy)
Contract Length: 13 weeks
Job Details
- Setting: Home Health
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts)
- Patient Ratio: 1:1 (up to 6 patients per day; 30 productivity points standard)
- Start Date: ASAP
- Weekend Requirement: No
- On-Call: No
- Floating: No
- Block Schedule: Not accommodated
- Locals Accepted: Yes (within 50-75 mile radius)
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have own transportation
- Travel Radius: Up to 60 miles per day (60-90 minutes total drive time)
- COVID-19 Vaccination: Primary series plus one booster required
- Charting System: EPIC Remote Client (laptop), Rover (phone app), and Oasis documentation required
- Attire: Business casual; scrubs allowed but no specific color required; no jeans
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Qualifications
- License: Oregon Occupational Therapy license required
- Experience: Minimum 1 year of skilled home health experience (higher experience requirement applies if conflicting info)
- Certifications: Basic Life Support (BCLS) required
- First-Timers: Accepted (no prior travel experience required)
- Oasis Experience: Required (please disclose if not experienced)
Prior Travel Experience

Compliance and Documentation Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- E-Verify attestation required
- Education verification required
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Drug screen: 10-panel within 90 days of start (instant tests not accepted)
- Immunizations and health documents required:
- COVID vaccine or declination
- Flu vaccine during flu season or declination
- Tetanus/Diphtheria/Tdap within 10 years or declination
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
- Hepatitis B vaccination series or signed declination
- Varicella vaccination or positive titer or declination
- Respirator fit testing required annually (travel only)
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment application covering at least last 5 years
Training and Competency Requirements
- Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Annual regulatory requirements
- Specialty-specific modules for Home Health and Therapy/Rehabilitation
- Competency exams as applicable by profession
- Medication test/pharmacology competency exam as applicable
- Skills checklist updated annually (if applicable)
Additional Notes
- Traveler may be subject to Providence float policy (may be required to float to other Providence facilities within reasonable distance)
- Facility requires photo ID badge (passport style photo with neutral background)
- No weekend or on-call requirements
- Locals within 50-75 miles considered local hires
This position is ideal for Occupational Therapists with home health experience seeking a travel contract in the Portland, OR area with a 13-week assignment length. First-time travelers are welcome.
